Your appointment:
If you are a new patient, it is important that you arrive 30 mins. before your appointment time, to complete the necessary paperwork.
Minors (age 18 and below) must be accompanied by their Parents / Guardians.
Please bring your Insurance Card, an Identification card (e.g. Driver’s License), your
authorization number (if needed by your Insurance Company) from your Insurance company.
Your Copay / deductible payment is due at the time of the visit. You should check with your Insurance Company regarding your payment responsibilities.
Please review our office Policies and Procedures and other forms
needed for the appointment prior to your visit.
You must print and fill out the forms and Patient Questionnaire
listed with this website and bring the forms with you for the
appointment.
For Adult patients, they must fill out Page 1-3 of the
Initial Questionnaire and the Mood Disorder Questionnaire.
For Children and Teenagers (up to age 18 yrs), the
Parents must complete Pages 1-4 of the Initial Questionnaire and
the ADHD Rating Scale (Home Version).
All Patients must print and fill out the forms for
Demographic information (the Patient Information form), the
office Policies and Procedures, the form to communicate with you
PCP, the form regarding Protected Health Information (HIPAA
form)
Please remember to bring with you all the above mentioned
forms with you at the appointment to save your time in the
office. Also if you have any previous medical records, school
reports, copies of Psychological testing you must bring those
papers with you. Please do not fax these reports to our office
prior to your visit.Forms:
